Jobsite Hazard Assessment (JHA) Form

Hi guys,

I am revising/developing our hazard assessment form for site inspections and for mostly liability reasons I'd rather not start from scratch. Can anyone recommend a good source for this type of document template? Most of the documents I have seen are more geared for laborers and not as applicable for engineers and short duration inspections.

Charlie, I think that "not as applicable for engineers and short duration inspections" is a potentially dangerous mindset to have. It only takes a split second for an accident to happen. This opinion is based on my time spent as a laborer, materials technician and an engineer. If you let your guard down because you're only on site for a short period of time, you're asking for trouble. When I'm on site for an inspection I am constantly looking for hazards. I am not trying to imply that you're unsafe in what you're doing, just my initial thoughts based on your opening statement.

As for a JHA...see link below (page 39). PCL is hands down the most safety orientated contractor I have ever worked for. Hope this helps.

of a 192 page document...not a criticism on PCL, but it is a shame that it takes 192 pages to describe the Project Health, Safety, and Environment Plan. Who actually reads these documents? I am pretty sure the laborer doesn't. Probably just the original author/s, and the attorney's when something goes 'south'.

Yes, I think it is a sad day when the chaff in a safety document takes more pages than the project technical specification. But hey, if that's what makes you happy...

The 3rd page of the PDF indicates that this document is intended for the Company Owner, Superindentent and site Foreman or each contractor/subcontractor...not each individual worker on site. I have been on many PCL jobsites and this is the first I've seen a document like this. I came across it searching for their JHA template that the OP asked for. Everyone on a PCL site (anyone from laborer to engineer) must take their safety orientation before going to work. This consists of going over their safety policies and going over their hazard assessment template that I referenced above, among other things.

Hokie66, with all due respect, I think your comment in misinformed. I can't find the technical specification for this project, but I guarantee you that it is longer than 192 pages. The document I linked to was for government project...In Canada, on a project like this, I would expect the job specifications to be 500+ pages long. I wish I could find a link to back that up, but I don't have the time right now.
