Does anyone have a reference book or dvd that can be recommended to teach someone how to make a LOTO plan using P&ID's. This may be a combination of P&ID tranings as well but ultimately my end goal is to be able to mark up a P&ID for any piece of equipment found on a P&ID for a chemical processing plant. Any assistance is appreciated.

A P&ID may not give all the information you need to safely lock something out. Electrical energy, energy in springs and weights are a few things that come to mind.

You need to tag the valves and gives numbers which need to be put on PID's

make sure that all the valves (whether control, on-off, relief, isolation) whatsoever the size is should be tagged. apart from this i do not think there is LOTO things you need to include in your P&ID. LOTO procedure is usually a standard safety procedure available with all companies and also mentioned in the safety pages of the equipment operation and maintenance manual.
