Meshing with CTETRA 10 - Failed elements

Hello,

I am meshing a CAD model with CTETRA 10 elements with Midnode method Linear to avoid Failed elements. Afterwards I use 2D Mesh in certain areas for refinement with Midnode method Mixed. Then I still have Failed elements especially regarding Edge Point Included Angle.
I would like to ask you for help.

In the CAD model
(pull down) Menu -> Analysis -> Examine Geometry
all the checks should be toggled on, then select the model with a rectangle, and press "Examine Geometry" on the bottom
