Shield wires are not very effective at lightning protection on distribution lines because of the low BIL. A good proportion of lightning flashovers are because of induced voltage from nearby strokes instead of from direct strokes to the line. If the shield wire is struck, back flash, from the ground wire to the conductor caused by the surge current through the ground, is more likely with low BIL.
Putting the neutral on top complicates connection of pole mounted transformers.
More pole height is needed because you need higher ground clearances to the lower conductor.
If you had secondary on the same pole, you would want it below the primary. This would require additional pole height.
