Does anyone know of an equation to calculate the diameter of an orifice in a thin-edged orifice plate for a particular K-value (loss coefficient)? In particular, for water or slurry duties.

You might consider a wedge meter for your slurry service application. It is more suited to these parameters than a traditional dp device. There are several reputable supplers available.
