It's an interesting listen, but I imagine an all too common problem.

FWIW, in the nuclear industry, whistle blowers are extremely well protected by federal regulation. Any retaliation against someone reporting a safety concern would be met with extreme punishment by the NRC.

I know that's not the case everywhere. Just look at the Challenger o-ring problem.
