Hey can one list reasons why PRV/PSV fails to open??

See one reason like chattering, It affect seat. similarlly wat are the other reasons to fail other Parts.??

Chattering may occur when the relieving fluid capacity is much smaller than the PSV orifice capacity. The disc lifts, pressure is quickly relieved, the disc closes and excess pressure accumulates below the valve. This chatter can be very fast with high energy levels that quickly wear the disc and seat. Where suitable, pilot operated valves may eliminate this problem.
 
Section 5(CAUSES OF IMPROPER PERFORMANCE) of API 576 is a good place to start
 
Chatter may result from any one of the following reasons:
1) PRV Oversized for installation; System Flow is < 25% of PRV Capacity (ref JLSeagull comment)
2) PRV Inlet Piping Excessive length (Pressure Drop > 3% of Set Pressure)
3) PRV Inlet Piping undersized ("Starving" PRV)
4) PRV Outlet Piping excessive length (Built up Back Pressure)
5) PRV Outlet undersized (Built up Back Pressure
6) Two Ring Design Safety Valve (ASME Sec. I Boiler Service) Upper Ring too high (PRV Huddling Chamber Geometry fails to sustain Lift)

Other Failure modes include:
1) Incorrect Set Pressure
- Fail to compensate for Temperature and/or Back Pressure
- Adjusting Ring out of position (induces simmer)
- Misalignment (lateral Binding induces friction)
- Compression Screw Locknut Loose
- Pressure Surge causes PRV to lift below Set Pressure
- Bellows Valve, Bonnet not vented
2) Seat Leakage
- Loss of Product, Fugitive Emmissions
3) Long Blowdown
- Reseat below operating pressure (Process upset)
4) Excessive Simmer
- Longer than blowdown, PRV hangs open and leaks

Several years ago I was a student in an Instrument Safety training class. When talking about reliability, the instructor stated a "safety valve will fail to operate when it is suppose to operate about once every 90-years". This is based on yearly testing. I can not vouch for the analysis - just repeating what I heard others say. The previous responses are all very good in listing why safety valves fail to work. In my experience rupture disks add lot of unreliabilty to safety valves not working when they are suppose to work. Everyone knows about the impact of pressure between the disk and valve inlet. What is not well known is the (+/-)manufacturing tolerence of the disk. I have seen disks with(+)7% tolerence and the safety not operate when required. I have also seen the safety not work because maintenance folks installed the wrong disk. Good Luck
 
hello all i m really sorry but little bit busy with something, had problem in plant and busy whole day with that. i guess all responses are very help full. i will look API-576. Thank you JLSeagull, JAlton and Yitbos. these reasons are very worth it. hey guys u can add more reason when ever u want.

Is Spring Bonnet opening makes difference? means if its open to atmosphere?? and if its open to manifold header?
 
Yes, the Spring Bonnet Opening can cause a Failure. if the PRV has a bellows the bonnet must be vented. This permits air to leave the bonnet when the bellows is deflected during a PRV lift. If the bonnet is plugged the two elements exposed to the pressure created by the deflection of the bellows are the very thick bonnet and the very thin bellows, hence the damage to the bellows and a resulting failure of the PRV.
