Requirements for Foam System

I have read through NFPA 11 which provides information to design a foam system for storage tanks (ie. number of nozzles, flow rates etc.). What is the criteria used to originally determine if a foam system is required or not. Are there guidelines based on the combustibility and volume of the tank contents? Is it based on requirements of the underwriters? Any information is appreciated.

Read through NFPA-30 and understand the various categories of combustable and flammable liquids.

Then evaluate the spacing/siting requirements for the various types of tanks.
