Rerouting and existing building to reconnect to an existing building sewer 6

Good day Eeveryone,

There is an existing building sewer that all house utilities drain into, as shown in green. We need to reroute this line since the existing sewer main at the rear of the house will be abandoned, and we plan to connect the building sewer to the new sewer main at the front of the house. This existing building sewer is located below the slab ground, making it very difficult to overhaul the entire plumbing layout.

My question is: would it be acceptable to install three 4-inch, 45-degree PVC elbows with a double cleanout afterward? The elbows will be located under a thick pavement, which limits us to placing only a double cleanout outside the house. We are located in the Commonwealth of Northern Mariana Islands, but I haven't found any applicable local plumbing codes.

If this is not a suitable option, I’m open to any suggestions.

It is not preferred, but sometimes you just have to make things work, so you try to eliminate the risks, or provide solutions to easily repair the risks. Then discus it with the future asset owner as they are the ones that are going to have to deal with it.
I think you would need to have a cleanout on the green secton as well to allow you to rod/jet from both ends. most likely location of a blockage is in the bend

Why not build a chamber at the turn? That turn looks very of and can't really be swept from upstream.

A chamber would allow you to do both.

You could lay half pipe 45 degree bends in the base.

Thankyou all for your Reccomendations @swazimatt , @LittleInch , & @bimr ; Apologies for the late replay.

Following the recommendations Please see the image below and I look forward to further comments from you guys:

Questions :
1.) Does IPC Allow double clean-out and one of the clean-out in this shop drawing is oriented upstream/opposite of flow.
2.) Will Option 2 be better than Option 1?
3.) Will Option 2 require a vent as shown in the shop drawing? Based on IPC.
4.) Will Option 2 be allowed to use the Wye T for the drop pipe connection or a 45 degree elbow and a Wye should be used? Based on IPC.

I recommend option 1.
Thank you for your recommendation. Am I correct in assuming that the IPC allows a cleanout in the upstream direction? May I request that you share with me which section of the IPC Code states this?
