Residential 24x 30 Cocrete Foundation

I am attaching pics of floor wall opening that was created when walls were poured. This foundation is on cape cod abutting a 100 yr flood plain.

The foundation was open for 3 years before house framing. Has anyone seen anything similar? There were 2 of these openings on rear wall, one has been filled.

When you click on the upload to the engeering.com site, a widow pops up to which you can download your information. Did you use this window and follow the instructions?

I doubt anyone will give you their email address here, directly at least. Kinda against the rules...
