Square D Power-Zone Switch Assembly

I have encountered a situation with a Square D Power-Zone Switch Assembly, nameplate picture attached. My big concern is that this is installed on a 13.8kV system but the switch is rated at 5kV.
My questions are:
1. Is it possible that this switch could have been manufactured as a 15kV but mislabeled?
2. Is it possible for the switch to be converted to 15kV and the nameplate not updated?

I am doubtful on either one as the fuses on the nameplate and spare fuses at the facility are 5.5kV rated. I don't know if the existing cabinet could support increase in clearances needed to go to 15kV plus I would think a field retrofit would cost more than replacing.

I don't know the history but the transformers fed from these switches have a 1990 date on the nameplates but I don't know the year of this SQD equipment. I hoping that some people with more SQD experience may be able to clear this up as I find it astounding that this could be the case.

Is this switch on the secondary of a 13.8:480 Volt transformer?
The 9f62dcb200 is listed as a 5.5 kV fuse.

This is on the primary side, 13.8kV comes from the utility pole into this switchgear with 3 fused bays, each feeding a 13.8kV transformer primary, the secondary is 208Y/120
 
I believe that 5kV & 15kV load break switches are the seem switch but the fuses are, of course different. Square D calls that switch HVL (high voltage load).
I do not have the Sq D Digest with me now, but I remember that bays are about 3'+ wide.

Note the BIL Level listed on nameplate is 95kV.
Pretty standard on 13.8kV equipment.

 
Only differences in parts book for these are for 600 or 1200A and some differences based on fault ratings. so think the 5 kV and 15kV are the same switch besides the nameplate info (And fusing)
