The Chemical Safety Board has released an updated animated video on or about the 15th anniversary of the BP Texas City refinery 2005 explosion and fire.

An ad for CSB's forthcomin interactive training application takes up the first 40 odd seconds of the video. The animated narrative starts at 00:43.

The animation is quite realistic, including a coffee mug stain in the control room log book. It's just like you're there on-site or watching it unfold from a drone. Gives me the shivers.

Believe there is at least one important contributor to this incident at BP refinery in Texas that wasnt pointed out in this CSB video, which is a basic process design flaw: The safety valves on the isom unit splitter tower ovhd line at the base of the tower lifted prematurely at below setpoint pressure at the tower top. That was because the set point was set at design pressure and was not designed to handle a liquid release which contributed some several bars of static head since the tower was liquid full. Hence when the PSVs' lifted at SP, pressure at tower top was well below SP. Those 3 safety valves should have been installed at the top of the ovhd line, not at the bottom.
Sounds like there was flow instability in the transfer line from the isom unit reboiler back to the tower, which may explain why these operators habitually overfilled the tower bottoms sump on startup - thermosyphon reboiler?? Another cost driven design approach which compromises operational flexibility??

The CSB is a very small group. They serve as project managers on these investigations. Their funding and existence is attacked from every angle. Some people want this rolled into an agency with enforcement powers. Others attack them because "all" that they do is produce information.
Their work has saved thousands of lives over the years as professional learn from past mistakes and then engineer solutions.
Whenever you have a chance promote and defend the work of this agency.

The U.S. Chemical Safety Board, which has only one board member instead of the usual five, on Friday postponed a scheduled public meeting that was to include discussion of its plans to operate even as President Donald Trump pushes to shut the watchdog agency responsible for probing major chemical accidents.


“Three of Donald Trump’s trips to Mar-a-Lago are equal to the CSB’s entire budget. Which is a better deal for the American people?” asked Wright, citing government spending records.

This is yet more evidence of how a mature civilized society comes apart when the party of the rich gains power and could not care less for the average man .....

And that isn't even a drop in the bucket. This is the worst time in western history for collective knowledge and democracy since Galileo's trial. , and maybe more to the point, the dismantling of the Magna Carta. And that's not even a drop in that bucket either. The principles of a society based on truth, justice and democracy have never been so offended. I trust my German friends today will agree .. The average German of the days leading up to WWII believed life during that period (1933-Dec 7, 1941 Pearl Harbor) was Germany's most glorious times. They could even holiday in Spain. Then reality struck. As it did in Rome, Berlin and no doubt countless lesser known societies in history, the path to tyranny begins when good men (and women)) do nothing. VOTE!
