Is there any update / root cause identified on the earlier thread i.e. "Crack in flange side when weld Flange CuNi to Tee CuNi".

Root cause could be anything like below average welders, poor welding practice, the surfaces to be welded were not cleaned properly or the composition of flange was not as per specification. This has to be done by site personnel only.

I think probably the issue is they used Lincoln consumable for something important.
They make good for SAW and fence building.

Also WeldStan's answer from that thread I agree with:
"With the very low heat input, LOF may be the actual flaw"
 
Now, now meltedEng,
I haven't seen Lincoln trying to hype the abomination known as self-shielded wire in a number of years.
And they have upgraded themselves by acquiring decent companies like Techalloy and Metrode. (If you can't beat em', buy em'.)
