Back Ground:

A welder welded 10 tubes in which one tube rejected in visual test and two in macro examination but other 7 tubes accepted in visual, PT and macro examination.

Question:

Is welder qualified as per QW-193.2 " a minimum 05 tubes are required for qualification".

Why did the welder weld 10 tubes instead of the required 5 tubes ?
You have 5 x acceptable so theoretically the welder has passed.
However, the code provides minimum requirements - it is up to you as the representative of the employer to decide on acceptability of the welder.
3 x DNC from 10 x welds - a 30% failure rate. Not acceptable.
This is only my opinion but if I was in your position I would have the welder weld a further 5 x tubes and all 5 must pass.
 
Agree with DekDee. Make sure that the welder sees the welds that did not pass and explain to him/her why the welds did not pass.
 
I suggest further training or practice and then new test.
