In an industrial plant,how is comprehensive utility piping (water, Steam, Compressed Air...) documented and identified?

Examples are welcome.

Thank you, but I need to make my issue more clear:
I work in a slowly (50years) growing factory where utility piping he been expanding also.
We have come to a point where there is no clear view of the situation anymore.
So we need to make some sort of diagram to make clear how the piping runs. So eg: when a technician as been called for a leak, he can asap find out which valve to close.
Some-kind of P&ID but without control loops and a minimum of instruments. I don't need details about materials etc.
Only Pipe/manual valve/gauge... identification, connections,location...?
How is such info achieved/documented.

The utility system is typically documented in the same manner as the process piping when a facility is being designed. That would include a P&ID showing all of the controls and instruments. You would also have a line list showing materials of contruction.

The plant should prepare as-builts when the utility system is constructed and then keep the drawings up to date.
 
That's certainly the ideal. The reality is that you do as well as you can, given the resources and available information. This is more about finding where the original information might be hidden. There may just wind up some pipes being labeled "IDK" (i don't know) except that SOMETHING is moving in the pipe.
