Worm Drive Hose Clamps violation example

In thread thread190-378290 it was mentioned that someone was looking for an example where a company was cited for using worm drive clamps on compressed air hose.

That thread was locked, so I had to start another to post an example. On page 17 of this 2013 report the company visited was found to have four locations with worm style screw clamps and proposed penalty of $5500.00:

I still think its BS though..
Took me 3 days to redo every connection in our entire facility with the double "pinch/crimp" rings.. (Yes.. I'm Engineering and wear the maintenance hat also when needed)

I asked the inspector to show me why those rings are "approved for airline/barb use" and the worm drive clamps aren't.. He couldn't... but could only say that in a power point presentation as part of his training it was shown that the worm drives aren't allowed..
