Lately there have been talks on a PCA publication related to achor bolt design.

Try also to check on thread592-4469 in The Concrete Engineering forum (or simply do use the search utility in Eng-Tips as follows: Search - keyword "anchor bolts"-all forums and go)where a similar question was posed and several references were given by pals here who are almost experts on the subject.

I found the materials here very helpful: AISC's Design Guide 7: Industrial Buildings: Roofs to Column Anchorage / Fisher (1993)

There is a very good design procedure outlined in the Uniform Building Code, 1997 under Section 1923.

Also, some of the concepts and principles can be found in a publication by Nelson Stud Welding Division.

Personally, I use the AISC LRFD bolt design under combined loading to design anchor bolts. As far as the anchorage strength of the concrete, I apply the 1997 UBC provisions as well as the provisions in the latest PCI design handbook.
