California High Speed bridge problem - Madera

Has there been any recent news of a bridge failure on the California High Speed train in Madera, California? The LA times has run a story concerning this and our local 3 day a week newspaper (Mt. Democrat - August 28th, page A4, written by Dan Walters covered this) Happened last October. Stop work issued November 4, 2019.

Please keep in mind that this incident occurred while the bridge was being constructed, not while it was in service, which it isn't yet.

It was 'failure' in terms of corrosion of bridge components - namely 7-wire PT strands within ducts, due to water ingress, during construction.

Tutor Perini responded to the LA Times in a letter: “The bridge in question, just one of more than 30 bridges in Tutor Perini’s contracted portion … had an issue with water in a few of the post-tensioning ducts. Through investigation, Tutor Perini discovered that the water made its way into the post-tensioning ducts through broken vent tubes. These tubes were found to be broken during installation by the rebar subcontractor."

Rebar/PT subcontractor is on the hook for repairs/costs.

How do they know the vent tubes were broken by the ironworkers and not the pump hose during the pour, unless they had photos? If they had photos, why not fix before the pour?
