CMU Lintel Damage 1

I received an email with the attached photo from an architect concerning a CMU lintel that has cracked on the underside of the block (see attached photo). This is a solid grouted wall supporting a wood roof, should not have been a problem.

I must admit I have never seen a crack like this. The contractor is saying the cracking is from over vibrating.

Any thoughts?

Ok went and looked at the damaged lintel yesterday. I poked up through the cracks and there is grout. In a couple of locations there is a +- 3/4” void in the areas of the largest cracks.

The roll up door is supported by the jambs so that is probably not a contributing factor.

The rest of the structure looks fine.

The mortar was not finished on the underside of the lintel because of this issue I assume.

The construction is still on going and the building is not occupied yet.

I have requested a lot of information such as inspection reports and the roll up door specs so I can figure out a repair.

My hunch is this is probably damage from consolidation vibration and it is probably not much of a structural issue with this deep of a lintel but it is ugly, especially when inside the building looking through that door.
