I don't know that they were necessarily completely to blame. It's pretty clear that the protocol they were using was ineptly built, otherwise, the usage logs would have had a slot for entering the measured weight of each disposed explosive. The bottom line was weighing the explosive was almost only a suggestion, not a mandate; like the Challenger disaster, the protocol worked, until it didn't
