Hi everybody,
Is there any guideline available for writing an Engineering Failure Analysis Report, by institutes such as API, ASME, ASTM or by an experienced person?

IME the report is just another technical paper, nothing special. Conducting a failure analysis OTOH differs from traditional engineering enough that formal training is highly recommended. Given that much of what you're reviewing/analyzing will be process, having your own clearly defined is critical to prevent backlash.
 
I suggest you look at some of the material presented by forensic engineers like Brady Heywood, particularly his discussion on root cause, and high reliability organizations.
These organizations are chartered with finding root causes, so their reports are not biased to assign blame NTSB, U.S. Chemical Safety Board (CSB).

CWB1 is completely correct about failure analysis being very different from traditional engineering.
