I have attatched to screen shots of a lifting frame I am working on...

I am struggling on how to fix it?

One screen shot shows it fixed through the lifting hole using a 'Fixed Geometry' fixture and a 'Distributed Mass' on top of the box section

the other screen shot shows it with a 'Fixed Geometry' fixture on top of the box section and a 'Bearing Load' through the hole

Which is correct? or please could you point me in the right direction if these are incorrect...

How are you assuring that the loads are in balance? If you are putting a distributed load on the box frame does the resultant pass through the hole? If you fix the box frame you may be creating a situation that is not what you are trying to model. Since there is so much deflection are you sure you don't need to consider the loads following the deformation?
