Where is that requirement in the code? For example 1 to 6 slope. Thank you.

I don't think it's in the code. Looks in the PtI post tensioning manual if you are in the US.

I am trying to find an electronic view of this.
 
PTI manual gives some direction. They give 1:6 slope or 10m radius if I remember right. I called VSL with a similar question and they thought the 10m radius was fine
