User Defined Symbol 1

Can someone lead me through step by step on how to create a User Defined Symbol similar to the "Triangle with point Up" that is in Insert>Symbol>ID symbols, so that I could then use that in annotation text as a flag symbol with a number inside?

I am using NX 5/6. The documentation is kind of sketchy as to the procedure.

I attached an image of what I think the end product should look like. I inserted the triangle and moved it over the text. I tried to attach it to the text but it is not cooperating.

The best approach, without having to create a bunch of new symbols, would be to do what you're already attempting by placing an existing 'Triangle Up' ID symbol over the top of your note (it's best to toggle OFF all of the 'Alignment' methods and just eyeball-it), but then I would go one more step and create a 'Group' which would contain both the original note as well as the added ID symbol(s).

Once you have these items in a Group, you can still edit the text as before, but if you need to drag the note to a new location or hide it or edit it's color or whatever, you can perform these tasks on the entire Group as if it were a single object. There is however one thing that you will need to remember, and that is that you will have to actually select the Group and not it's members if you want the convenience that this approach provides. Now there are two ways to select the Group; the first is to just set your Selection Filter to 'Group' before you select, but the second way is much better and and lot quicker. What you need to do select any object which is a member of the overall Group and then go to the 'Selection' toolbar and select the 'Up One Level' icon which causes the entire group to be selected. Now to make this even easier to use, do what I did: I used Customize and added a keyboard short-cut to the 'Up One Level' operation, in my case I assigned it the letter 'U' (not Ctrl-U or Shift-U, just plain 'U'). Now all I have to do is when I wish to select the entire group, just pick something and press 'U', and continue.

Anyway, give it try and see if this will work OK for you.
