Utilities Placed in Concrete Slabs

I'm designing an industrial, one-story building slab that will require plumbing and electrical conduit placed within the slab. Is there any general guidance on how to embed commodities within a slab? I assume ACI has some restrictions on conduit size and location within the slab. I've never designed a slab with utilities running through it, so I am looking for any outstanding reference material that discusses key considerations and best practices for embedding utilities within a slab. Design experience would be very helpful as well.
