Yes, many "user" require testing at the MAP condition. We do it regularly.
We re-design the vessel with a "new MAWP" which is the MAP of the lowest pressure of head, shell, or flange, making sure of the stresses incurred at hydro-pressure.

Well, first, 2" pipe is 2.375" o.d. but it really doesn't matter.
i don't remember this being addressed in code. BUT, the gaps at welds must meet your wps.
Most wps do not stretch out to 7/16" weld gap.
The code specifies that COLD formed parts with excessive elongation requires heat treatment.
It does not mention that hot formed parts need heat treatment.
This is why most head manufactures do a lot of hot forming.
If the plate was normalized at the mill and then normalized during head forming and finally PWHT....i don't think you have to do any additional tests if the material test reports show it was normalized.

all the new engineers want to plug it in the computer and what is spits out is correct.
They have no idea what to change and when to change it.
You can have internally certified level II if tested and records kept by the company level III
You can have an ANST testedd level II.

you should have a U-1A form available to see which plant manufactured the vessel.
The CODE does not require any particular amount of radiography (except in circumstances of Lethal service, some materials, thickness, etc)
It does however, require that the stamping reflects the the design of vessel. It does not disallow extra NDE.

Apparently, your design is not adequate for the required MDMT. There are several things you can do.
I am assumining it is a carbon steel vessel and you don't give the materials you selected.
You can add normalization to the materials.
You can add PWHT.
If the problem is self reinforced nozzles...

We design our inner shells with external pressure induced by the jacket.
we test jacket at 1.3 X MAWP jacket.
we have never had the problem you are encountering with failure.
generally speaking....a davit eyebolt (w/ unc threads) is not designe to lift the blind. Threads are for slight adjustments to align the bolt holes.
if you are looking to raise a 2300# blind 4", I would look at an oversized eyebolt with acme thread and handles welded on the nut.

the pressure, whether map or mawp is at the top of vessel. the code requires that you design for all known loads. static head is a load on nozzles, especially those low in a liquid vessel.
