Interior Ramp Detail

I'm wondering how others detail interior or even exterior ramps.

Section at top of ramp:
1. Place small footing, foundation wall or turndown slab at top of ramp?
2. Dowel bars from both the adjacent SOG and the ramp SOG into the foundation wall (stated above)?
3. Create a construction joint at the intersection of SOG and the ramp SOG?

Section at bottom of ramp:
Same question as for the top although I think here you would either turn down the ramp SOG and the but up the adjacent SOG and place expansion joint filler between them.

I will try to attach one solution shortly.

Obviously, the small ftgs., found. walls, or turndowns must go in with the slab sections which go in first. And, the main slabs on grade may be the most important in terms of location, elevations and finish. Then put the ramp and its side walls in to match the primary slabs t&b. Fill, light compaction and screed off fill as a bottom form for the ramp. Reinforce the ramp with something more than just mesh since the fill under it will likely settle down over time, it is intended just as an underside form material.
 
Personnel (handicapped) only?
Or forklift and service trucks?
Will the ramp be covered with something to hide cracks and sags or dips?

In this case it is for Personnel however I'd be curious to hear different suggestions for forklift and trucks.

I'm not sure if the ramp will be covered or not. In this case it would probably be covered with carpet.

With forklifts or trucks I would treat it as a jump/approach slab and have the concrete ramp supported on elastomeric bearings at each end. Add some dowels or other means to prevent horizontal movement, but detail them to allow rotation of the ramp slab at both ends.
