Torsion or shear cracks and solutions

Have you seen anything like this yet? Is this indication of torsion or shear cracks and what should be the remedial actions? The beam is 12 inches wide and 20 inches deep with closed stirrups distanced 4 inches apart. And the location of it is in the following (shown in red):

This is zoom of the cracks (crack width similar to other cracks).

There is sufficient longitudinal, tranverse, torsion and shear stirrups inside. The concern is the beam has lost the rotational restraint (due to the cracks).. should steel bracket, etc be used to make it fixed again? What do you do in your experiences?

If the cracks are inclined and steep to be shear cracks..how would the inclined restrained cracks behaved (see my illustrations two messages prior). Remember the concrete already cracked so it would rely on aggregate interlock and dowel action only. The steep cracked is in stress. And what if it occurs between 2 adjacent stirrups?
 
By the way all the buildings designed by our company needs to withstand magnitude 7.3 earthquake bec most buidings are within 10 kilometers of such faults. How would beams with restrained shrinkage cracks behave? Still please tell me what would happen for cracks occuring between stirrups. Its not shear crack but would the longitudinal bars bent between the stirrups? Our company is not sure about this from lack of actual magnitude 7.3 experiences hence asking what you guys think.
